Community Forms and Documents are provided here for convenience.  Please refer to the appropriate sections on the left navigation tabs for details about the Documents and Forms.

The four online forms must be completed to reserve the clubhouse:

Clubhouse
1. Clubhouse Reservation Form:
Used to make reservations 

2. Clubhouse Rental Agreement:
A contractual agreement between the member (who is the renter) and Ellington HOA.

3. Clubhouse Policy:  Consent Form

Club House Walk Thru Checklist Form: (For your Review ONLY to be aware)
Used by board members or a person designated by the board to make sure the clubhouse
is in the same condition on the day before the rental and remains the same after the rental.

Violations
Summary Violation Guidelines:
A summary of items from the Covenant that are considered a violation.  

Violation Form:
Used by board members and committee members to issue 1st and 2nd notices of violation.

Architectural
Click here to download the Modification Form.
The modification Form is to request changes to the exterior of your home and lot.


Architectural Guidelines:
A document, developed by board members and authorized by the covenant that lists in detail
what members are allowed and not allowed to do when making exterior changes to their homes and lots

Modification Checklist Form:
Used by the Modification committee to ensure modification requests are in compliance with the covenant.

Community Assessment
Community Assessment Form:
Used by community members (such as yourself) to report any broken easements, broken entrance lighting, potholes, or any other visible and pertinent items throughout the community.
